Travel Certificate

Before attempting to travel with your pet, please remember to:

  • Visit the USDA APHIS Pet Travel website. This will help you obtain specific requirements from the government of your destination. Some destinations require several months of planning.

  • Obtain specific instructions from your airline or travel service.

  • Please email us the following information:

  • Any previous vaccination and microchip information for services that were not performed at BLVD Vet.

  • The name and address of the person who will be shipping the pet at the departure.

  • The name and address of the person who will be receiving the pet at the destination.
